Use this form to submit requests for any use of facilities to hold a class, group, event, etc.

*PLEASE NOTE: If you need any production elements outside of those listed here, please indicate that here in the "other" option and we will have our Production Team connect with you.

*If the event requires more than 4 sets of tables/chairs, please allow yourself time to set-up and tear-down for your event. Tables and chairs will need to be returned to storage spaces after the event.

Once your form has been processed, if an approval has been made you will receive an email to confirm approval. The email you receive will also state who your contact person is for approved event, along with contact information. Thank you!
